A wholesale supplier to supermarkets, Bestway Northern, has been fined £1 million and costs of £11,950, following a fatal accident where a worker acting as a banksman for a delivery, was fatally crushed during a routine delivery. The investigation revealed that the company had failed to:
- carry out a suitable and sufficient risk assessment for workplace transport activities,
- lacked a safe system of work for addressing deliveries and unloading procedures,
- provide adequate training and banksman, particularly in high risk environments.
